improper lane change
Georgia. I was pulling out into traffic onto a four lane highway on a day with little traffic. Once I got into traffic my passenger (who is a bit of a back seat driver) "checked my blindspot" and said "your clear". AND of course I wasn't clear and I hit another car in the next lane.
