What is the name of your state? NC

GParent is 103 y.o. and had done estate planning. No assets were  in her name...this was done many years ago...she had given everything to her daughter and grandson who had been keeping her. Unfortunately, both her daughter and son have died and she is the only living relative to them...hence the assets are in the process of coming back to her...She is currently in a resthome...I want to make sure I do anything I legally can to preserve her assets but frankly don't think there is much that can be done...any ideas or resources???
