Can't find the link plus questions

I have finished my parenting plan that I want to submit to the court to possible get it entered into an order but I cannot find the proper link for the paperwork to file with it... I have browsed through; http://www.superiorcourt.maricopa.gov/ssc/forms/alpha_list.asp and cann't seem to find what I am looking for. any help would be greatly appreciated. Also I have a few questions about our last mediation conference. Read as follows:

Mediator wrote "Whatever mutually 'agreeable' arrangements at which they may arrive, or alternatively, whatever orders may be entered regarding custody and parenting time, it is likely there will be continuing disputes between the parents that rise to crisis level, given that they will be dealing with each other on the matter of long-distance parenting time which will require advance planning, out of state travel for the children, and considerable financial expences for that travel. Unfortunatly, it seems necessary that an oversite mechanism be employed in this case to ensure as much as possible that Mother complies with any orders in place regarding custody and parenting time."

What is an oversite mechanism?

Also, Mother is requesting that she be allowed a 2 week period of vacation time with the children once a year. I cannot have this because it would be impossible for me to take my 2 week period without it falling on mom's time or the kids missing school. I would like to offer her the last 2 weeks of summer vacation instead so that it is a set time each year. Do you think this would be acceptable? I would be fine with this as I would still be having them for 4 weeks over the summer as they go to a year-round school and their summer break is only 6 weeks long. With the break being so short I do not see how I can only take the kids for 3 weeks and then return them to mom for her 2 weeks (I have to return them a week early so that they can prepare for school).

Is that confusing?

You can certainly propose it that way if you feel that's best. You would simply state that the schedule for summer break is the first three weeks to dad, the last two weeks to mom.
She may however not agree as she may want some flexibility as to when she takes her vacation. Most people would.

I have no problem giving her flexible date but I am confused how to go about that. According to our divorce decree we have to give eachother a 30 day advance notice of all parenting time that we wish to exercise. Tham means I have to notify her 30 days in advance that I will be exercising my visitation with the kids over the summer. With that information I HAVE TO submit to her a travel itinerary and my paid receipts for the airfare. If she only has to give me a 30 day notice of HER days and she takes them closer to the end of the summer then I will have to then pay to reschedule my flights as her 30 day notice will not be due intil after MY 30 day notice.

"Mother shall inform father of the period of her 2 week summer parenting time no less than 30 days prior to the beginning date, and such time shall not infringe upon father's previously arranged summer parenting time."

No I am looking for the paperwork that I attatch to my parenting plan that I would like considered by the court. We went to mediation and could not come to an agreement there and I basically took what the mediator suggested in his findings in regards to my visitation and elaborated on it. I know that I have to include the family court cover sheet but what other paperwork would I need to add? Shoudl I just type up a "Petition to Enter into Order Proposed Parenting Plan"?
