What is the name of your state?What is the name of your state?New York. My friend just had a baby. She is not married to the man she had the baby with. She gave the baby the fathers last name. Now she is sorry and wants to give it her last name. Does the father have to be contacted about the name change?

Is the father listed on the BC as the father? Is the father involved?
