What is the name of your state? Oregon

What form do I use to dissolve my conservatorship and show where my mothers money went?

I read your other thread. In about two weeks, you'll be looking at being in contempt of the court. You need to get your butt down to the probate division of the courthouse and get the papers to file a formal accounting. As IAAL stated in your other thread, you better be able to show how you spent momma's money -- every penny of it on her care.

After your accounting is approved by the court, then you can get the papers required to be discharged as your mother's conservator and obtain your court order discharging you. You must keep all records for a certain amount of time after your discharge order and need to know that too.
