What forms are required to apply for conservator. My mother would like to apply for conservator for her mother. She is currently housed in a care facility and has been diagnosed with dementia. She is aware of the fact she will need an attorney to complete this whole process however time is of the essence as her brothers son currently holds poa. Is there a way my mother can inniate the proceedings immediately? Our main concern here is that the brothers son is not appointed without my mother knowledge. He is nothing more then a pawn for his father as his father could not be apponted due to his criminal background. I think his heart is in the right place but the father has to much influence over him.
Can she be appointed conservator of estate as well as person?

Since your grandmother is NOT in an unsafe place, emergency conservator does not apply. Have your mother see an attorney asap and ask about Temporary Conservator which could be applicable in this situation until the formal conservatorship hearing is held.
Can she be appointed conservator of estate as well as person?
Yes. Also include health care surrogate/proxy.
