What is involved if I wanted to create merchandise with either the name or the logo of a high school or a college team?

Copyright infringement unless an agreement or assignment of copyright is obtained. License fees, Intentional Tort actions against you from the original copyright holder or licensee....shall I continue?

I understand all that. What I meant was is the license agreement with the actual school, or school district or at a higher level.

As far as college, is this something licensed by the individual college, a governing body over the school or the NCAA?

That is the question, not if I would require a license.

No one is going to walk you through the complicated process of obtaining a license which can be different for each school. Your best advice is to sit down with each school you are interested in and discuss the matter with them.
