What is the name of your state (only U.S. law)? Texas

Nearly 8 months ago, I had surgery. It took four months or so, but Blue Cross Blue Shield finally paid all the assorted claims. Yesterday, they sent me a letter stating that they had decided that one of the tests they already paid for was not medically necessary and demanding that I reimburse them for the payment.

So, my questions are:

1) is it kosher for them to do this - and to do it months after the fact
2) what can happen if I ignore their letter. Note that they can't cancel my policy because it was already cancelled (for non-related reasons) as of 12/31/2015.

Thanks in advance for helping me to figure out how to deal with this situation.
