I told my manager that I was moving out of state but not sure when. My recent evaluation was excellent. Everyone in the company got a raise except for me. The manager told an assistant manager I didn't get one because I was leaving, but I haven't given my two week notice and it might be months before I move. Any advice???

Your employer has no obligation to give you a pay raise and if they decide not to do so because you're a "short-timer," that is perfectly legal. You've announced your intention to leave even if you haven't given them a specific date yet.
