What is the name of your state? Ohio
There was a fall recently where I work. I documented in nursing notes and also filed an incident report. Suddenly, my nursing notes have disappeared as well as the incident report. The facility was at fault. No major injuries, however I am concerned that my nursing notes disappeared. If investigated, how do I prove that I did indeed document?

Computer or paper?
Does your facility have a risk management division?
The missing documentation may be in review or under "lock and key" pending further investigation.

I'm sure it was destroyed to cover. It's a small business, no risk mgmt. Pages in nurse notes were arranged as such that the entry before mine is there, and the later entries are there. Only mine has been wiped out, so that there is no mention of the fall. I have heard that the family has a lawyer. They did call in an ombudsman to investigate, but the "higher ups" are excellent at fabricating stories. I would hope that the ombudsman has not finished her investigation since no one has talked to me about it.

Have you contacted the ombudsman ?
