What is the name of your state (only U.S. law)? Kentucky

Good Afternoon!
I am in the process of resigning from my current employer. I have provided a verbal 3 week notice, and have not sent any written communication as of yet. My employer is requesting I sign a form resignation letter that involves waiving my rights. I am not sure how this form benefits me in anyway...and I definitely do not want to sign anything regrettable. Thank you in advance for your help - copy / paste below removing company name.

I, ______________________________________ (print name), do hereby voluntarily resign from employment with Company with my last day worked as (date).

I understand that I forfeit all rights and claims to employment with company.

That is actually fairly standard. It doesn't benefit you in any way, you're right; but it doesn't hurt you either, unless you think you have a valid right to sue them for something. If you truly think you have a valid lawsuit or if there are any bonuses that have yet to be paid out, don't sign it. Otherwise, it's fairly neutral and is SOP.
