Explicit Content Forms - Where can I find them?

What is the name of your state? New York, California, Washington, Ontario


Hi.

I host a Variety Show in various cities in North America.

The content is very Explicit. I want to make sure I get this Warning across.

I was wondering if anyone knew of a place that had ‘General Warning’ Forms that I can edit and place on the front doors of the Venues where I run my Show from.


Other information:
- My Shows are run out of bars and lounges
- I run the Show in front of 40 people or so

What kind of Explicit Content?
- Offensive/Vulgar Language
- Adult Scenarios
- Nudity
- Sexually Explicit Content


Does anyone know where I could possibly get a ‘General Warning Form’?
Or can someone point me in the right direction?!
