While you don't ask a question, if it is "can there be one", the answer is no. After the act is complied with it is all in the hands of the employer.

There is no such thing as a FMLA extension. FMLA ends at 12 weeks, period.

While the employer may, if he chooses, extend the medical leave provided by FMLA, the FMLA protections no longer exist starting Week 13, Day 1. Once again, the employer may choose to extend the protection. He cannot be legally compelled to.

If you're unable to return to work at the end of the 12 weeks of FMLA protected job leave, FMLA ends.

As cbg noted, the employer "may" choose to extend you additional time off but they are not required to do so.
