Health Care POA?

What is the name of your state? CA

My father made a Living Will in 1991, designating together my mother and us 3 daughters to make his decisions. He has since been diagnosed with Alzheimer’s. He needs cataract surgery next month, the dr has said mom must have Power of Attorney for him to receive the surgery. Is it Health Care POA she needs? The General or Durable POA does not seem like the right one. Can we fill out and file the form ourselves, and go before the judge, or do we need a lawyer? And where can we find the Health care form?
Is there anything else we should be doing, such as Advance Health Care Directive, or Conservatorship? These are some terms I’ve come across in my research. Thank you very much for any help you can give us.
