Help?? k1 visa questions

1.I am from US my fiance is from UK. He is 43 with a heart problem. Had 1 mild heart-attack 3 yrs ago. He can work and is on med's now. Is this a prob?
2. He lived in South Africa in early 80's and needs police certificate. What is the Identity number they are requesting and is this necessary being so long ago?
3. He also lived in France on a temporary visa. What is needed to get the police certificate there?
4. On the k1 petition form it asks Family name, I was married, do i put my maiden name here or my married name?
5. On the G-325A form it asks for the File number, where do I get this number from before mailing into INS?
I am very concerened about these issues especially the heart issue. Could you please respond as soon as possible.
