Help me file a declaration

I need help finding out how to submit a declaration to the court, Where do I get the paperwork? I am appering Pro Per.

Also... I have subpoenaed day care records from the petitioners mother ( I was paying 800.00 a month) And we have been told that there are no records- no tax returns, no reciepts, nothing. Can I file something with the court to get the money that was supposed to be used for child care back? It totals 19,000.00. I have filed a complaint with the BBB and the Petitioners mother is still will not comply--- Please Advise.
