I work for a medical group for a couple of years...i have 4 weeks off, sick days, and personal days. I usually get paid by the hour and now that is changing to per-patient. Does that mean that my empolyer has the right to cut my vacation time and everything else?

Unless you are operating under a contract that specifically says otherwise, your employer has the right to change the vacation and other paid time off programs whether the pay method changes or not.

Why would they? I presume they'd like to retain their physicians and other skilled employees. Have you asked your employer what their plans are on this? The odds are significant that all that will change is the manner in which they compute your actual compensation, not your fringe benefits.
