How do you enforce medical bill payment without an attorney?

In the divorce decree the ex spouse was ordered to pay half of all medical expenses. A letter has been sent to ex and is aware of all charges. Ex refuses to help pay on said bills. The balance of all medical is around $1000.00 some have been paid in full and others are on payment arrangements. I called the local court and they said to have an attorney file the proper paper work. I called an attorney but they charge $500 for the procedure. I filed my own custody and support papers in the court and represented myself which was successful. I would also like to represent myself for medical collection. I need to know what is the name of the form or type of form that I need to file to collect said bills. I live in the State of Tennessee. I am aware of the procedure but just need to know the name of the legal form. And how it should be worded. The court secretary told me it would fall back to the original divorce decree in the Chancery court. Any help is appreciated.
