What is the name of your state (only U.S. law)? Florida

I need to draft a 1099 agreement (as an alternative to full time employment) for an organization here in Florida. I will be vending internal technology support as an independent contractor. Can anyone point me to samples that I can rely on to draft this document? Google appears to yield information mostly on 1099-MISC forms. Thanks.
