What is the name of your state (only U.S. law)? CA

My ex is over 20k in arrears. I have attempted to negotiate with him, but he quit his job, will not get a new one. He recently received over 20k in a settlement (lawsuit) and did not pay either the back taxes (as ordered in the divorce decree) or the child support.
What forms do I need to fill out to file for contempt of court for both of these issues?

One of your local courthouses should have a self help center or a family law facilitator's office. That is the best place to get forms and assistance in filling them out.
