my husband received a questionnaire from his employer requesting personal medical information, it states that it is an injury fund questionnaire, it asks such questions as what medication you are taking and what current medical problems you have, as well as have you ever been treated for etc etc etc, medical issues as well as mental., several of my husbands coworkers think this information should not be required, is this a violation of the Hippa Privacy? on more than one place it states in bold/"failure to answer this truthfully and completely, may result in termination. PLease let me know what the law says about this, this is a privately owned company. :
