What is the name of your state? California/Colorado
I am filling out the paperwork to file for contempt of court charges against my ex who was supposed to pay me a maintenance fee of $3000 after he got his divorce decree last August but reneged and then told me he had only said he was going to just to get me to agree to an uncontested divorce. I live in California and he lives in Colorado. I am filing out Colorado form jdf 1816 to start with... Anyway, in filing out the form, I am wondering if I list myself under my married name still since this is what is on all the divorce records or do I use my maiden name now since the divorce court granted me my request to restore my former name. I don't want to confuse anyone or fill the forms out wrong so what name should I list?
Also, the part on top where it wants you to fill out "Attorney or Party without Attorney", do I fill out my name in that spot since I am the person filing this document or does my ex's name go there since that's what's on all the other divorce related records?
