What is the name of your state?OH
On the living will form, the first option:
I do NOT want to live being totally and permanently dependant on one or more machines or an artificial life support system for continued life.

It states you may cross out all that you do not wish for. Does this mean that say there is no brain damage, but maybe coma, I am signing to not receive support as needed. Of course, the rest cover other aspects, but this wording is not totally clear to me.
Anyone know?

That wording can be so interpreted as such. There's a lot more that needs to be included in such a document:

* Designation of Agent - medical POA (with alternates in the event the agent cannot be reached).

* Agent's authority - all medical decisions, decisions for forms health care to be provided to keep you alive. This includes providing/withdrawing artificial nutrition and hydration.

* When Agent's Authority Becomes Effective.

* Agent's Obligation.

* Agent's PostDeath Authority - organ donations, disposition of your remains.

* Nomination of Conservator/Guardian.

* End-Of-Life-Decisions - prolong or not prolong life.

* Relief From Pain.

* Effect of Copy.
