I currently live in South Carolina, my divorce was finalized in California and I want to change my married name back to my maiden name. In SC the divorce decree has to be written that my former husband will consent to this. Is there a legal form that has to be filled out to accomplish this?

My response:

If you are currently residing in California, merely file a Petition for Name Change. The Petition will be published in a newspaper of general circulation and, if there are no legitimate objections to the Petition, then you'll have a short hearing on the matter. The judge signs the order for the name change, and zimbo, bambo, your name is changed.
