If you work at a hospital and the hospital has adopted a policy saying that when you call in sick you have to tell them your symptoms, is that legal?
I know that if you go to a doctor, and get a note saying you can't work for so many days, they can't put down why, so why would it be legal for the hospital to ask it's employees what their symptoms are and document that if they were to call in sick?

Yes, because of the nature of your job, it would be legal. The purpose would be to protect the health of the patients and staff, which may otherwise be compromised by your illness.

Have you looked at the forms that the government suggests using for FMLA, for example?

There are circumstances in which the employer is ENTITLED to at least a certain amount of medical information. There is no law that unilaterally prohibits a doctor from providing it.
