Medical release letter??

What is the name of your state (only U.S. law)? IN

My 15 year old son will be picked up by his uncle today and will be staying with him at his Grandparent's home for a week. His father will be picking him up from there the following weekend for his summer parenting time.

Should I send a letter stating that they have my permission to authorize any medical attention necessary while my son is in their care?

If so, what information should it include?

I just want to make sure everything is covered.

How far away are you? If its a really long way then it would probably be a good idea. Do a good search for medical POAs.

He will be about 3 hours away. I would hope that if there were an emergency there would be no need for them to have a POA but they do Paintball and hunt and stuff like that. I'm just concerned if something minor happens would they need a POA even if they had the insurance card to have him seen.

I will research Medical POA's.

I include on my POA all the pertinent information they ask you when you register. Parent's names, addresses, etc. When you are stressed is NOT the time to try to remember any of that. I include a copy of the insurance card.

I found an example of a POA form online. It seems to be in order and has all the basic info that would be needed. I just added the insurance information on the bottom.

When he does Scout stuff, they have this real basic form that is signed by the parent and I have always sent him with another letter from me with more information just in case.
