On Schedule I and J the income and expense forms do I need to put a snapshot of the current month or an average month .

Take all your expenses for a year and average them out, put down the resulting number. Things like electric bills vary by season, so you must average it.

Income - use the numbers from your tax return and your YTD for this year.
