What is the name of your state? TX

Is there a special form I will need in order to have someone in OK be my poa agent for both personal and business purposes? I have a medical condition that causes confusion and lack of concentration; so I want something in place in case I am not able to take care of my own affairs for a period of time up to indefinitely.

First of all, you need to an examination to determine whether or not (due to your confusion & lack of concentration caused by your medical condition), that you can make such decisions. If so, then have an attorney draft your POA and any other estate papers.
