Parents names on birth certificate?

I've heard of stories about people finding out that they were adopted after finding their birth certificates... But, are there instances when the birth certificate is amended to reflect the adoptive parent's name instead? So, basically, when you look at the birth certificate, you can't tell that there was an adoption. Is this common?

So... This person grew up with and was raised by his birth mother, and his (adoptive) father, who adopted him after marrying his mother. The weird thing is that this father's name is on the birth certificate, not his birth father.

What would be an explanation for this?

When a child is adopted a new birth certificate is issued, with the name of the adoptive parent(s) and the original birth certificate is either sealed or destroyed by the state. You cannot tell from the new birth certificate that the child was adopted.

The reason why some people find out the names of their birth parents, from birth certificates is because a copy of the original birth certificate happened to still be in someone's possession. Sometimes the adoptive parents even have a copy of the original.
