What is the name of your state (only U.S. law)? Alabama

I was divorced 3 years ago. My divorce decree states I own the home and my ex-wife was to sign the quitclaim deed. She moved away (West Coast) before she signed it and up until now she refused. My financial situation my cause me to need to put the house for sale in the next year. Due to this she has agreed to sign the quitclaim deed. Where can I get the form? What specifically should be filled out, etc. Thanks in advance.

I am also trying to due a HAMP loan modification. Is the quitclaim deed needed to do this?
