What is the name of your state (only U.S. law)? AZ

My mother-in-law discovered that she has developed arthritis in her knee. She discovered this by walking with my son when her knee began hurting. Unsure if it was an injury or what happened she eventually went to the doctor who gave her the news. She deals with it now and is coping fine. However, the Rawlings Company sent her a letter urging her to complete an online questionaire. After a little research it appears they are acting on the behalf of her insurance company who is attempting to transfer the payment for her medical services elsewhere. Questions like date of the injury and where the injury occurred lead us to believe that if the pain began at someone's home they'd begin to look toward's the homeowners to foot the bill. Is this the case with this company and how should this situation be handled especially if there was no injury to report?

This is normal. If there was no injury, simply report that there was no injury and no third party responsible for payment.
