Received letter from OC after dispute on CR, help!

I received the following letter from an OC after I disputed it from the CR agencies. Please note that this account was referred to a few CA, and they Validation letters were sent. Please advise me on what to do next... Thanks in advance:

Dear Me,

this letter shall serve as acknowledgement of your recent notification regarding the above refereced account, which was purportedly acquired without your knowledge.

We have included an affidavit that we request you sign and return. For the affidavit to be valid, please sign it in the presence of a notary public before returning to us. If you are unable to obtain a notary, provide a legible copy of a valid state issued identifcation card. To proceed with the processing of your claim, please return the affidavit notarized or the signed affidavit along with a valid state issued ID in the pre-addressed envelop provided within 20 days of receipt.

If we do not hear back from you within 40 days, we will assume that you have decided to withdraw your fraud claim and the charges will be placed back on to your account for billing. If the account remains delinquent, you could receive collection calls, and your credit history could be impacted.

Upon our receipt of the information requested above, we will notify you in writing of our actions regarding this claim within 30 days.

Once the investigation concludes that fraud has occurred, we will request that the credit bureaus remove our information from your credit report. Please note it could take the credit bureaus up to 60 days to do so.

I'm not familiar with your history. Was this debt fraudulent? If so, then simply fill out the affi, have it notarized and send it back as soon as possible by CRRRM. Be sure to make a copy of it before sending for your own records.
