What is the name of your state? new hampshire

I have just finished 12 weeks of salaried continuence after a major surgery. My doctor gave me a note to return to work on temporary reduced hours and my boss is now telling me that I must take an unpaid extended leave of absence, fill out an ADA accomodations form, because of the reduced hours, and file for disability insurance. I was under the impression that disability insurance is for people with permanent disabilities and not for those who need to work a limited schedule. Please clue me in...

There is no law that says disability insurance can only cover people who have permanent disabilities. Many, many, many disability policies cover people who have only short term disabilities; thus the term, short term disability insurance.
