What Forms are Needed for Custody Change?

What is the name of your state? OK & TX

All parties are in agreement of one of our two children moving between states. So that I don't have to pay our lawyer (and since we've already financed his boat and new wardrobe), do y'all know if there's a simple form we can fill out to change custody from OK to TX? Our son is 16 and will be moving over the next weeks and we want to make it official and change custodianship (and child support, I suppose).

Joint Custody (50/50) between both states, child currently resides in OK. All custody preceedings previously have been held in OK.
