What is the name of your state (only U.S. law)? San Francisco, CA


Where can I obtain a name change document that contains the legal name both before and after the name change? I think I was given this doc when I was sworn in to be a US citizen but I misplaced it. I need to renew my passport and it requires to produce such a doc.
