I live in Virginia and purchased a house after I was married. My husband is not on any of the documentation related to the home. I am the only person on the mortgage. I am not sure what documents he should be added to so in the event of my death he retains the house. The mortgage is something I do not want him added to (ideally) because his credit is poor compared to mine and I have a low interest rate. We also have a daughter if that impacts the process at all. Thank you for the help.

There is a simple solution to the problem. You simply leave the house to your husband in your will.

Put the house in a trust if you'd like. There is no need to put him on title now. Love is not always forever and having him on title, but not on the mortgage, is something you will regret if it ever gets to that.

If it is left to him in my will and I still am paying towards the mortgage at my time of death, will he need to reapply for a mortgage?

That potential problem exists whether you leave it to him in your will, you put it in a trust, or you add him to the deed now. However, in general the mortgage companies don't take any action as long as the mortgage continues to be paid.
