Boss forged sig on legal documents

Im posting this for my girlfriend.

She works for a adult entertainment company that is very popular. In order to work in adult entertainment you have some govt. compliance forms you need to fill out. She was only told to scan her driver licence and sign and date a piece of paper and that was it. Well now she is not working for the same studio and went to switch studios (same major company, but they have the studios hire the models)
And she has 8 forms to fill out as part of the compliance, and she was told she allready filled them out beforehand. She gave attention to the corporate offices of the main company and they said in fact the sig's do not match, but now they will not respond to her and now not let her work on the site anymore without any cause.
Is there a way we can get her boss in trouble for signing forms as her, because if he did this for her, he did this for all his other models or at least a good amount of them.

Since you say that these are 'Government Compliance Forms', have her file a complaint with the appropriate agency.
She can also contact her local police and see if they will take a forgery complaint.

forgery
n. 1) the crime of creating a false document, altering a document, or writing a false signature for the illegal benefit of the person making the forgery. This includes improperly filling in a blank document, like an automobile purchase contract, over a buyer's signature, with the terms different from those agreed. It does not include such innocent representation as a staff member autographing photos of politicians or movie stars. While similar to forgery, counterfeiting refers to the creation of phoney money, stock certificates or bonds which are negotiable for cash. 2) a document or signature falsely created or altered.

Nevada Revised Statutes:
NRS 205.095 Other acts constituting forgery.
Every person who, with intent to injure or defraud, shall:
1. Make any false entry in any public record or account;
2. Fail to make a true entry of any material matter in any public record or account; or
3. Forge any letter or written communication or copy or purported copy thereof, or send or deliver, or connive at the sending or delivery of any false or fictitious telegraph message or copy or purported copy thereof, whereby or wherein the sentiments, opinions, conduct, character, purpose, property, interests or rights of any person shall be misrepresented or may be injuriously affected, or knowing any such letter, communication or message or any copy or purported copy thereof to be false, shall utter or publish the same or any copy or purported copy thereof as true, shall be guilty of forgery and be punished as provided in NRS 205.090.

NRS 205.110 Uttering forged instruments: Forgery.
Every person who, knowing the same to be forged or altered, and with intent to defraud, shall utter, offer, dispose of or put off as true, or have in his possession with intent so to utter, offer, dispose of or put off any forged writing, instrument or other thing, the false making, forging or altering of which is punishable as forgery, shall be guilty of forgery the same as if he had forged the same.

Here is another problem I did not state in my initial post, Her old boss runs his company from Maryland, and at no time has she ever had any "real life" contact with her old boss, so would this be a automatic contact of the correct govt agency that would handle this?
