My question was concerns about liability. It is a self help book. What kind of disclaimer should I put on the book?. Should I incorporate myself? and then have the publishing company sell my book on Amazon?

Risk management is entity choice, business practices and insurance. Of the three in your circumstance, insurance would be the most important thing you can do now. (Assuming the book is properly vetted by an experienced editor.)
