I am from IL.
I have a 5 yr old son. I never put anyone's name down for the father on the birth certificate. Well I have just now recently married my son's father and we want him to have his last name. So that we all have the same last name. What is the procedure to having this done?

If you never established paternity with the father you need to go to the local child support office and for a small fee they will have the two of you sign a paternity statement and file it with the court. This will legally establish him as the father, then take the order to your vital statistics office to add him to the birth cert.
