If former employee forged my signature on his application for professional license

Why do you want to pay to do the Licencing Boards job?

It sounds like you notified them via some sort of online form. If you are this revved up over this pick up the phone and call them.

Generally licensing boards will require a form be filed with an outline of the concern.

Fine but calling the people that are issuing the license and telling them that one of their applicants has forged one of the application documents should surely get them off high center.

That might move to the top of the pile the form that is filed. :)

At this point, there is only a suspicion that a signature was forged. The recently-licensed fellow might have come by his license honestly.

In fact, because there IS only a suspicion and no proof, I warn 151515 against mentioning his suspicion to others.

Since all of this is based on getting a license that I assumed the OP would have the license as well and know what and who had to sign the forms to get the license and new that he didn't sign it.

He said he only suspected forgery of his signature. It could be someone else's signature.

But I don't know. We don't know what license was issued. :)

I called them and they advised that I need to do it in writing. They do not disclose any info over phone.

It’s HVAC license. It could be someone else’s signature, you are right. I want to find out that mine was not forged. If someone signed it for him I don’t care, that’s their business.

Keep in mind that you have only heard that this person has been licensed. It is entirely possible that the person was not telling the truth to your employee about being licensed. And if it is some sort of board monitored occupation, electrician, blaster, surveyor, etc. they will have the mechanics in place to examine any reports of fraud in experience, even if they did not verify the experience at the time the license was issued. That's surprising, as usually they tend to expect people to lie to them about things like this, and usually verify all required experience that is self reported by the applicant. Anybody can make things up and fake a signature.

Finding out if the license was acquired with (or without) your signature should be relatively easy. It will be what to do with the information if it is discovered your signature was forged that will be the difficult part. :)

Not only heard, I checked on the Secretary of State website, he has a license, it’s listed there.
