What is the name of your state (only U.S. law)? PA
I need a document to prove that I have been covered by my insurance for the last 5 years. I requested a certificate of coverage but instead of a start date of coverage it states "at least 18 months or prior" and then end date of the coverage.
I asked them to put the actual start date of the coverage or at least month and year but they refuse and say they will not provide it. I asked to speak to the supervisor but they say she is not available and even if she was she would tell me the same thing.
I asked if there was a law or a rule stating they can't do that and they keep saying that there is no rule or law but they still will not provide me with the date.
That seems unreasonable since I have been paying for the coverage along with my employer and I feel that I should be able to get the start date in writing. I could be wrong of course. Can someone tell me if there is any way I can get the start date out of them?
