Legal name change

What is the name of your state? IA

After a divorce, I changed my name back to maiden. On my daughter's birth certificate it shows married name. Can I have that changed to show my maiden name? If so, how do I got about doing this?

You can send a certified copy of the judgment in your lawsuit to the folks who issue birth certificates in IA and that should change your name on your daughter's birth certificate.

Ask them what form you need:
Iowa Department of Public Health
Bureau of Vital Statistics
Lucas State Office Building, 1st Floor
Des Moines, IA 50319

This question is just out of curiosity:
I have always heard that before a parent could legally change a child's last name, they would have to get the other bio parent's permission to do so.
Have I been misinformed?

She isn't asking about changing the child's last name....she is asking about change HER last name (the mother's last name) on the BC.
