Legal Name Change

I didn't know where to post this. I want to have my name changed on my Driver's License, so maybe the Traffic Law board may have some help. I also posted this on the Family Law board. FreeAdvice.com really should have an Identification Law board. Any help is great, though...

I recently spoke with the Civil Filing Office of Louisiana. I want to legally change my name and have no idea why it's so damn difficult. There are no forms to file, I just have to go before a judge. However, the man at the Civil Filing Office said I have to have something "drawn up and signed by a judge" so he can process the paperwork. What am I suppose to have drawn up? Am I suppose to write a little letter explaining why I want my name changed and have a judge sign it so that it's okay? I'm just changing it for preference. I don't mind keeping my old social security number. I'm not running from the IRS, or any credit card companies. I'm not married or divorced. It's just a personal preference. What am I suppose to "draw up?" Do I need an attorny to go before a judge? If anyone has been in a similar situation, any advice is greatly appreciated.

There are some great, easy to read and understand, books on this subject at your local bookstore. Go to their "Legal" section, and take a look. Most, if not all, of these books have the necessary forms with step-by-step instructions.
