What is the name of your state? CA
paternity has been established for my husband's son, how do we go about getting him put on the birth certificate and have the baby's name changed to his? does the mother have to agree?

how does the father get records

my husband was awarded sole legal and physical custody but the mother refuses to provide any info. He need the child's shot records, medical info, ss#. how can he get this info?

We live in NC but we just went through the same thing. Eventually, we were able to get his mother to help us change the name but at first she said no. You have to file a "Prayer for relief" through a Special Proceeding in Superior Court. That is what we were gonna do before we got her to agree. I don't know if it is the same in your court, but I'm sure there is a way that you can find out. Look at other court cases about the same issue or just look in the legal reference books at your local library. You should be able to self file this motion. You can also look on the internet for sample filings and copy them.

Wanted to add

Sometimes, depending on the childs age and the county you live in, you can have the last name changed to the fathers surname once he is added as the biological father if you take it to the local Register of Deeds office.
