Calif.amend name change

I live in the state of California. On Aug. 21 I went to court to make a change of name and wasn't sure if the judge would make any changes so I hesitated to ask any questions. My question is that I would like to add a middle name; is this possible at this point without going to court again? Someone suggested that I type at the top of the signed Decree " Amendmended" and file it with my middle name. Is this true or would I need to refile and go through the whole procedure again, with the newspaper and all? I also thought about writing the judge and asking if this would be possible. Please let me know the correct thing to do. I filed the original petition for name change so I did not think about this until the day before the court hearing. What do I do? If this is possible what forms do I use?
