I had a POA that I made in Wyoming. I am since changing the person that I named and my personal information. I now reside in Oregon. How do I amend form, (besides changinging persons name and my new information). Who, or to whom do I send ammended forms to after making said changes. I am disabled and need someone to act as my POA. I would appreciate any help with this issue.

To make sure everything is done right, do it all over again in Oregon and tell your lawyer you want language in the new POA that revokes all prior POAs. Then tell the former POA that he is no longer needed.
