So I'm needing to file a Proof Of Claims and I had a couple a questions about the form. Quick back story, I purchased some merchandise (Less than $1,000 - which had to be ordered) from a local store a few months back and they recently closed and filed for bankruptcy without ever fulfilling my order. So here I am filing the proof of claim as a creditor.
Questions:
Item 2, Basis for claims - what should be entered here? It says to see reverse side, but it's a 1 page on-line form. I currently have "Goods sold" as I found that as an option on an older version of the form
Item 3 & 3a, Last four digits of any number by which creditor identifies debtor - I'm guessing this should be left blank?
Item 4, Secured claim - I'm pretty sure this would be unsecured in my case?
Item 5, Amount of Claim Entitled to Priority under 11 U.S.C. §507(a). If any portion of your claim falls in one of the following categories, check the box
Specify the priority of the claim.
-Domestic support obligations under
11 U.S.C. §507(a)(1)(A) or (a)(1)(B).

...but not sure if I should be selecting any thing here.
