Hi,
I was in a car accident a couple of weeks ago. I was the victim of a hit and run driver in broad daylight and rush hour traffic. There were witnesses and I found out a week later from the DA's office that the driver of the other vehicle was caught and thrown in jail for dwi, failure to control speed and failure to stop and give info. Both of us are insured motorists. I am receiving therapy and have missed worked. My auto insurance company sent me a form to give them authorization to retrieve any and all information that my medical, employer, and educational organizations have on me. I prefer to settle the claim myself because litigation would be lengthy and costly. What information should I specifically authorize them to have? Can I create my own authorization form releasing only certain information? If I refuse to sign their form will I not be compensated? Can I limit their information only to the specific dates of the accident and medical treatment I am receiving for it? Any guidance would certainly help. Thank you.

depends. Do you have a pre-existing medical condition or a history of complaints similar to the injuries you are claiming from this accident?

What EXACTLY does that mean?? Are you saying that you waiving your claim for insurance coverage??

The request for authorization is very common in an accident claim.

Of course you can, with repercussions as noted below.

If I refuse to sign their form will I not be compensated?
If you refuse to provide the information required to justify your claim, it is likely that the claim will be denied, or reduced as unsubstantiated.

Can I limit their information only to the specific dates of the accident and medical treatment I am receiving for it?
Yes, but again, you will potentially be limiting your claim. For example, if they can't verify your employment and lost wages, you will likely NOT be compensated for them.

Any guidance would certainly help.
Then simply sign the forms.
