unpaid insurance premiums

My husband's employer has been witholding close to $700 from his payched monthly for insurance premiums. I received a letter in the mail stating that our family's insurance policy has been cancelled as of 2/28/05. I called the insurance company and was told that the insurance premiums had not been paid. Over the past year, our doctor visits have come back unpaid by the insurance company with an explanation (on the explanation of benefits form) stating that insurance was not in effect on the date of service. Additionally, several of my husband's paychecks have bounced over the past year.

Any recourse?

In addition to the other issues, I believe witholding pay for insurance premiums, then failing to remit those funds to the carrier, brings the state DOL into the picture.
