What is the name of your state (only U.S. law)? Colorado

I was asked by my lawyer and the GAL over my daughters case to write up a parenting plan for my daughter after a few months of the temporary plan. I was asked by them to ask my therapist to help me in wording of Joint Legal Custody/Decision Making as my daughters father, since we went back to court in October of 2009, has not involved me nor told me of any major decisions with my daughter - so they wanted me to word something for the Joint Legal Custody of who has final say over this or that, and divide it up - so that he can no longer keep me out of the loop of the major decisions. My therapist said she doesn't have any idea of how to do this - and wanted me to try and get some help from other people..... so can i get some ideas from anyone? Basically want to feel empowered again after all thats happened the past almost 2 years, and not let him bully me anymore, and have more of a say in my daughters life, than he has allowed. Thanks.

His wife provides the medical/dental/vision since she works, and he doesn't (hasn't since he got custody of my daughter in March of 2009)
